INNOVATIVE MINIMALLY INVASIVE AND TECHNOLOGICAL APPROACHES IN MODERN UROLOGY – CLINICAL EXPERIENCE, INTERDISCIPLINARY ASPECTS AND SCIENTIFIC TRENDS, Petar Radiev

Abstract:

As a result of technological innovations and the pursuit of more gentle, minimally 
invasive procedures, modern urology is developing extremely dynamically. Over the past two 
decades, there has been a transition from classical open surgery to endoscopic, laparoscopic and 
robot-assisted techniques, supported by precise imaging methods and digital technologies [1–5]. 
These changes lead to less significant surgical trauma, shorter hospital stays, fewer complications 
and faster recovery of patients [6]. 
Our review shows that urology is one of the most rapidly developing specialties in the technological 
direction: 
1. 
Modern urology is in the process of a profound technological and methodological 
transformation, in which minimally invasive and digitally assisted techniques replace classical 
open surgeries. This trend is clearly visible in both national and international clinical experience 
[7–39]. 
2. The integration of endoscopy, laser surgery, 3D visualization, MRI and digital planning systems 
provides higher accuracy, reduces trauma and improves safety for both the patient and the medical 
staff. 
3. In oncourology, the application of multiparametric MRI and endoscopic laser resection leads to 
better staging of oncological diseases, fewer repeated surgical procedures and an individualized 
approach to treatment. 
4. Minimally invasive approaches for BPH and urolithiasis show better results compared to 
traditional surgeries, have a shorter hospital stay and a lower risk of complications. 
5. In the field of functional urology and andrology, close interaction with other specialties 
(psychiatry, endocrinology, reproductive medicine) is necessary, taking into account the influence 
of medications and environmental factors on urogenital health. 
6. 3D technologies and radiation protection are among the most significant new directions that 
ensure safety, precision and modern standards in urological training. 
7. The development of urological practice in Bulgaria is in line with European and global trends, 
with local experience demonstrating potential for innovation and adaptation to global 
technological progress. 
8. The next logical step in the development of the specialty is the full integration of personalized, 
digitalized and multidisciplinary urology, based on evidence and safety.
